Embodied Heart Healing Arts Policy Notifications
I appreciate that you have chosen Embodied Heart for your inner healing and empowerment! These policies apply to all services and sessions. For your initial appointment you will be asked to sign in acknowledgment that you understand and agree to the following policies. Thank you!
Cancellation Policy
I respectfully ask that you provide me with a 24 hour notice of any schedule changes or cancellation requests. Please understand that when you cancel or miss your appointment without providing a 24 hour notice I am often unable to fill that appointment time. This is an inconvenience to me as your therapist and also means my other clients may miss the chance to receive services they need. For this reason, you will be charged 50% of the service fee for the first missed session and 100% of the service fee for each session after that. I also reserve the right to require a credit card number to be given to book future appointments so that appropriate fees may be charged if a late cancellation does occur.
I understand that emergencies can arise and illnesses do occur at inopportune times. Late cancellation due to emergency, illness, or inclement weather will generally not result in any missed session charges, but this is determined on a case-by-case basis.
Late Arrival Policy
Thank you for being on time! If you are early, I request that you please arrive no more than 10 minutes prior to your appointment time. I understand that issues can arise that may cause you to be late for your appointment. However, I ask that you call/text to inform me if this ever occurs so that I can do my best to accommodate you. Appointment times are reserved for each client; oftentimes, I cannot exceed that reserved time without making the next client late. For this reason, arriving after your appointment time may result in loss of time from your session so that it ends at the scheduled time. Full service fees will be charged even when sessions are shortened due to late arrival. In return, I will do my best to be on time, and if I am unable to do so I will add time to your session to make up for my late arrival or adjust the service charge accordingly.
Inappropriate Behavior Policy
All services at Embodied Heart Healing Arts are for therapeutic and relaxation purposes only. There is absolutely NO sexual component to the work whatsoever. Any insinuation, joke, gesture, conversation, or request will otherwise result in immediate termination of your session and a refusal of any services in the future. You will be charged the full service fee regardless of the length of your session. Depending on the behavior exhibited, I will file a report with the local authorities.
Embodied Heart Healing Arts General Liability Release Form
By signing below, you understand and agree to the following:
· I give my consent to receive services from Embodied Heart Healing Arts LLC which may include sacred space holding/massage therapy/energy work/Soul Coaching/Breathwork/therapeautic sound and any service I book for myself.
· I understand that the services provided by Embodied Heart Healing Arts LLC is not a substitute for traditional medical treatment or medications.
· I have clearance from my physician to receive these services.
· I understand that while my therapist will do her best to tune into my body and intuit my needs, it is my responsibility to inform my practitioner of any discomfort I may feel during the session so that she may adjust accordingly.
· I understand the risks associated with massage therapy include, but are not limited to:
short-term muscle soreness, superficial bruising, exacerbation of undiscovered injury. I therefore release Embodied Heart Healing Arts LLC and Kay Allmand LMT from all liability concerning these injuries that may occur during a healing session.
· I understand the importance of informing my practitioner of all medical conditions and medications I am taking, and about any changes made to these medications or conditions. I understand that there may be additional risks based on my physical condition.
· I understand that I or the practitioner may terminate the session at any time.
· I have been given a chance to ask questions about the session and my questions have been answered.
